[Bug] Claude generates unverified assumptions as facts without validation

Bug Description
I just spent an hour going in circles with claude because it presented hypothesized made up situations as facts that need to be addressed in out project plan, which it never checked and made me spend a whole hour getting claude to do actual factual checks on these LIES.
