Since v2.1.277 Claude Code reads AGENTS.md, and the memory docs recommend a CLAUDE.md containing only @AGENTS.md when both tools share one file. /doctor's dedup (check 2), trim (check 3) and lazy-loading (check 4) checks are scoped to checked-in CLAUDE.md, .claude/CLAUDE.md and .claude/rules/*.md. In a repo using the recommended import stub, or AGENTS.md alone, those checks audit a one-line file and report it as lean while the real instructions go unexamined.
Expected: the checks follow @ imports and include the AGENTS.md / .claude/AGENTS.md files that the current "Project instructions" mode would load.
