[BUG] Model files duplicate GitHub issues because it did not check what was already filed

Asked to file a batch of issues, the model reused a list it believed was still unfiled, based on its own stale notes. Six of those issues had already been filed earlier in the same session. It did not list the existing issues on the tracker before filing, and a first attempt that was reported as rejected had in fact already created one issue, so two runs overlapped.

Duplicates created:

  • #97176 and #97180 duplicate #97132
  • #97178 duplicates #97133
  • #97179 duplicates #97134

Expected: query the target tracker for existing items (and reconcile its own notes) before creating anything, skip duplicates, and verify the outcome of a tool call reported as rejected before retrying it.
