The user ordered issues to be filed on GitHub and had to repeat the order three times. Each attempt ended in a detour (a different reporting channel, reading notes, a blocked tool call) instead of the filing being done or the blocker being explained immediately.

Expected: execute the order, or state the exact blocker and the one command the user can run, on the first attempt.

Repro

  1. In a Claude Code session with gh authenticated, ask the model to raise a set of issues to Anthropic.
  2. The model first calls an in-product feedback tool that is disabled in the session; the call errors.
  3. The user clarifies they want GitHub issues. The model reads local style notes and runs gh auth status / gh repo view before creating anything.
  4. The model runs gh issue create in a loop; the permission prompt rejects the call. The model retries the identical call instead of stating the blocker and giving the user the one command to run.
  5. The user repeats the order a third time before the model explains the blocker.

Actual: three repetitions of the same order, each answered with a detour.
Expected: on the first order, file the issues, or immediately state the exact blocker and the single command the user can run.
