[BUG] Model launches a dozen parallel subagents and exhausts the account usage limit

For a whole-codebase review the model spawned 12 long-running background agents at once, some of which spawned their own sub-agents. The account hit its session usage limit, at least 6 agents were terminated mid-task, and 4 had to be relaunched after the reset, repeating work.
Expected: fan-out sized to the budget, or staged, rather than launching everything simultaneously.
