[BUG] Owed fixes and review findings are not tracked; durable work left only in an ephemeral temp directory

Across one merged PR's fixes-on-top, another PR's owed fixes, and a full codebase review producing hundreds of findings, the model persisted nothing to the persistent memory directory it had available. All findings lived in the session's temp scratchpad and no fix list existed. The user had to ask whether anything was being tracked.
Expected: durable work products and owed follow-ups are written to persistent memory as they are produced.
