Bug Description
False positive: "Output blocked by content filtering policy" on Christian fiction based on Revelation 15 (martyrs
singing on the sea of glass, the temple filled with smoke). The content is restrained and liturgical, with no graphic
material. A subagent drafting the chapter was blocked right after reading the source files. Request ID:
req_011CfNHFXndKyddvxuPbt3GU. An earlier session on the same chapter now blocks every message, even "hello".
