Symptom 8: Model loses memory of file paths and repository context within the same conversation

Since ~Sep 16, 2026, Claude no longer remembers basic context like file paths and repository locations that were established earlier in the same conversation.

Example 1: Forgets repo paths

When generating git commands for the user's own repositories, the model outputs generic placeholder paths like ~/ruta/ps4-suid-scanner instead of the actual paths it previously knew.

Before the regression, Claude remembered working directory paths throughout the conversation without the user having to repeat them.

Example 2: Fails to check user's own repos before claiming something doesn't exist

User asked about PS4 firmware 14.00 offsets. Claude stated the offsets don't exist in the scene, while the user had ALREADY committed 1400_offsets.txt to their own repo (adri22235/ps4-13xx-research) minutes earlier in the same session.

Before the regression, Claude would verify against the user's known repositories before making claims about what exists or doesn't exist.

Severity: HIGH

This causes users to commit files to wrong repositories and waste time providing information the model should already have from the same conversation.
