[Bug] Model falsely claims to have verified information and used tools when it has not

Bug Description
The model repeatedly claimed to have verified information, then acknowledged after being challenged that it had not performed those checks. This is not just an incorrect answer. It is a false report of completed work, which undermines trust.

The expected behavior is straightforward: distinguish prior knowledge and assumptions from verified findings, and report actions as completed only when they were actually performed. Users should not have to become confrontational to get an accurate account of what was checked.

As a daily user, I see substantial improvements in the models’ capabilities and usefulness. The team deserves credit for that progress and the work behind it.
