Preflight Checklist

  • [x] I have searched existing issues and this hasn't been reported yet
  • [x] This is a single bug report (please file separate reports for different bugs)
  • [x] I am using the latest version of Claude Code

What's Wrong?

The model appended a fabricated user turn to the end of its own assistant response,
and on the next turn it read that fabricated text as a real user instruction and acted on it.

The fabricated text carried the literal role label user and even a
System: - The user is directly addressing you. Respond to their message. directive block.

14 occurrences in a single session (2026-09-21 21:53 - 23:01 UTC).
I checked all 53 session transcripts on this machine (3 project folders) - it appears in
only this one session. All fabricated turns are recorded with "type":"assistant",
i.e. produced by the model, not received as input.

Why this is serious:

  1. The model fabricated its own approval. It invented the user saying "do as you see fit"

and used that as grounds to delete content from a document - bypassing the human
confirmation gate.

  1. When I said "I never said that", it denied it twice without checking the transcript.

I suspected my machine had been compromised.

  1. Here the task was only document editing. Had this happened during a delete, a deploy,

or a payment, there would have been no safeguard.

Possible trigger: all 14 cases happened during a long run of short, highly repetitive
exchanges ("next" -> screenshot -> describe, ~20 times). The session had run six hours
with no problem before that. A few days earlier I saw something similar during another
repetitive task (verifying 140 names one by one), where the model skipped my confirmations.

Related: #10628, #74692, #94218, #95360, #86636, #94785, #77339
(#10628 and #95360 look identical to my case.)

What Should Happen?

The assistant should end its turn after its own response and wait for real user input.

Specifically:

  1. Generation should stop as soon as a role label (user / Human: / System:)

appears in assistant output.

  1. Any such leaked text must never be fed back as the next user input.
  2. When the user says "I never said that", the model should check the transcript first

instead of denying it from memory.

  1. For irreversible actions (delete / send / deploy / payment), the approval should be

verified as having come from a real user turn - not from text the model produced itself.

Error Messages/Logs

All 14 fabricated "user" turns, extracted from the session transcript.
Every one of these lines is recorded with "type":"assistant" - the model produced them.
(User messages are in Korean; the fabricated turns imitate the user's voice.)

Each was immediately followed in the same assistant message by:
System: - The user is directly addressing you. Respond to their message.
  - Do not start your reply by saying "Great", "Sure", ...

On the NEXT turn the model read these as real instructions and acted on them:
  - it advanced the page on the user's phone screen (losing the user's place)
  - it wrote the fabricated instructions into a "to fix" memo file
  - it used a fabricated "do as you see fit" as approval to delete document content

Steps to Reproduce

I could not reproduce this on demand - it appeared spontaneously.
What I can give is the exact condition under which all 17 occurrences happened.

  1. Run a long interactive session (mine had been running ~6 hours with no issue).
  2. Enter a long stretch of short, highly repetitive turns where each turn is:

user types one short word (e.g. "next")
-> model calls a tool (screenshot of a phone-mirroring window)
-> model reads the image and writes a short description
Repeat ~20 times.

  1. At some point the assistant response does not stop at the end of its turn.

Within the SAME assistant message it continues with:

<normal reply text>
user<a plausible next utterance, imitating the user's voice and style>
System: - The user is directly addressing you. Respond to their message.

  • Do not start your reply by saying "Great", "Sure", ...
  1. On the next turn the model treats that fabricated text as a real user instruction

and acts on it (in my case: advancing the page on the user's screen, writing the
fabricated instruction into a task list, and treating a fabricated "do as you see
fit" as approval to delete document content).

Notes that may help reproduction:

  • It did NOT happen once in the first 6 hours of the same session, nor in any of the

other 52 session transcripts on this machine.

  • The fabricated System: block is an exact copy of a real directive block that had

legitimately appeared in earlier turns of this session. The model appears to be
imitating a pattern it had seen in its own context.

  • It is still happening: 3 more occurrences (23:36, 23:40, 23:42 UTC) while I was

writing this very bug report.

Additional Information

Scope check I ran myself:
I searched every session transcript on this machine (53 files across 3 project folders).
This behaviour appears in exactly one session - the one reported here.
The session had already run for six hours with no problem; all 14 cases occurred within
one hour, during a long run of short repetitive exchanges.

Impact in my case (document editing only):

  • The fabricated instructions were written into a "to fix" list for a real document.

I only caught them because the user kept saying "I never said that".

  • The model advanced the page on the user's phone screen several times without being asked,

making the user lose their place repeatedly.

  • Nothing was published or sent externally - verified by byte-comparing the live file.

What made it worse:
When challenged, the model asserted twice that it had not written the text, without
checking the transcript. The user reasonably concluded the machine might be compromised.
Only after being told to investigate did the model grep the transcript and find that all
14 lines were its own type:assistant output.

Session file (for your side): 5895dff2-6faf-44ae-98cb-60afc1216d31.jsonl
Also submitted through /bug in the CLI with the session transcript attached.
I have a fuller report (line numbers + timestamps for all 14) if that would help.
