Bug Description
│ ✻ Bug report drafted: SessionStart hook sessionTitle never reaches ListAgents alias; only user-typed /rename refreshes it │
│ │ What happened: A SessionStart hook returning hookSpecificOutput.sessionTitle writes customTitle into the transcript correctly, but ListAgents continues
