[Bug] Agent tool intermittently unavailable, requires restart to recover

Bug Description
Very often Agent tool is not available. Restart claude and resume fixes. But WHY???
I also noticed that this most often happens after exiting planning mode (ExitPlanMode) with the option to clear the context.
