[Bug] Intermittent Slash Command Loading Failure

**Bug Description**
Claude Code CLI intermittently fails to load my slash commands

Intermittently, when I start Claude Code CLI it simply doesn't seem to know about my slash commands. To see them I have to /exit and go back in. The second time it usually works. This is intermittent, so it is hard to give a repro case. What I can say is that it is happening in my case for personal commands, but it might be the same for project commands.
