[Feature Request] Cursor/VS Code Extension - Add Configurable Working Directory for Monorepo Support

Bug Description
When using the claude code extension in Cursor/VS Code, I need a way to set the working directory. When developing in a monorepo, it's critical to run claude code in a subfolder rather than root everytime.
