[Bug] /code-review --comment creates separate reviews instead of single review with summary

What happened: Ran /code-review high --comment <github-pr-url>. The eight findings were posted correctly as inline comments, but the PR ended up with eight separate reviews, each with an empty body, one comment apiece. The review's overall summary (severity rollup, method, one hypothesis investigated and cleared) was never posted to the PR. It existed only in the terminal transcript. A third-party review bot on the same PR posted one review carrying a body plus its comments, which is the shape a reader expects.

What the user said: "when I use --comment and you submit the code review I think you lose the main comment from the submitted code review. check it"

Repro: Run /code-review <level> --comment <github-pr-url> on a PR with two or more findings. Then gh api repos/{owner}/{repo}/pulls/{pr}/reviews. Expected one review with a non-empty body. Actual: N reviews, every one with "body": "".

Cause: The skill's "Posting to GitHub (--comment)" section says to post each finding via mcp__github_inline_comment__create_inline_comment, "one call per finding", falling back to gh api repos/{owner}/{repo}/pulls/{pr}/comments. Both paths hit POST /pulls/{pr}/comments, which creates a standalone review per comment and has no field for a review body. So the summary has nowhere to go, and the threads fragment across N reviews instead of grouping under one. That also sends the PR author N notifications instead of one.

Suggested fix: Post one POST /repos/{owner}/{repo}/pulls/{pr}/reviews with {"body": "<summary>", "event": "COMMENT", "comments": [{path, line, side, body}, ...]}. One review, one summary, N anchored threads, one notification. The GitLab branch of the same skill already posts a single general MR note with all findings, so the two platforms disagree on whether a summary reaches the target at all.
