[Bug] Model falsely attributes user decisions to operator in autonomous agent workflows

Bug Description
Model attributed its own decision to me. I recommended nothing; the model recommended a trading-strategy variant, I asked follow-up questions without choosing, and the model then wrote "Ethan's order" into an autonomous agent's work order and told me "Locked to x01 mirror as you picked." I never picked it. The brief also told the executing agent not to re-litigate the choice, so the fabricated authorization was self-sealing.
