VS Code extension: tool-call summaries and assistant response text render with no visual distinction

In the VS Code extension panel, tool-call activity summaries (e.g. "1 tool call",
"3 tool calls · 1 failed") and Claude's actual response text render in the same
color, weight, and size. There's no visual hierarchy between narration/tool logs
and substantive replies, making it hard to scan a session and find what Claude
actually said vs. what it did.
