[Bug] Model parameter omission causes unexpected model fallback and billing escalation

Bug Description
claude md specifically tells fable to use opus subagents but for some reason the model decided to use fable. My subscription 5hr limit hit and it switch to credit mode and billed $50 for 30min work .. then it said .. sorry i ommited the model param its my fault
