xhigh effort level no longer available on Opus 4.6

xhigh effort level was previously supported on Opus 4.6 but as of today it falls back to high. The /effort command now lists xhigh as requiring "Fable 5, Opus 4.7+, Sonnet 5" only.
Setting "effortLevel": "xhigh" in ~/.claude/settings.json is accepted and stored, but the session runs at high instead.
This appears to be a regression — xhigh worked on Opus 4.6 before today's update.
