Bug Description
The /code-review skill at "medium" level on a 68-commit branch forked into
8 finder agents plus ~10 verifier agents and hit the 5-hour session limit
(HTTP 429) mid-verification, before ReportFindings was emitted. The user had
stated they were at 30% of their window. Request ids
