[BUG] Desktop app slash menu lists every claude.ai account skill twice (bare name + anthropic-skills: prefix) since 1.40609.0 — no dedupe

Environment

• Claude desktop app (macOS): 1.40609.0 (auto-update applied via ShipIt on 2026-08-28 08:59 local; previous version did not show this)
• macOS: Darwin 23.4.0
• ~90 skills managed on claude.ai (Settings → Capabilities → Skills, "Save skill" flow) — no local ~/.claude/skills directory at all

Summary

Since updating to 1.40609.0, the composer's / autocomplete in the desktop app lists every account skill twice: once as the bare name (odoo-api) and once with the plugin namespace (anthropic-skills:odoo-api). With ~90 skills this doubles the menu to ~180 entries and makes scanning it much harder.

Both entries invoke the same skill — this is a presentation/dedupe issue, not a functional one.

What I could verify locally

• The dual registration is not new: local session transcripts from as far back as 2026-07-27 already show skills being injected under both identities (bare + anthropic-skills: namespaced, served from the ~/Library/Application Support/Claude/local-agent-mode-sessions/skills-plugin/<uuid>/<uuid>/skills/ bundle). So the data layer has carried both names for over a month.
• What changed is the menu UI: the pre-update app showed each skill once; 1.40609.0 renders both representations without deduping.
• There is no local ~/.claude/skills copy involved (directory does not exist), so this is not the local-vs-cloud duplication described in the workaround section of #90858 — here both entries come from the claude.ai account pipeline itself.
• ListPlugins for the account returns empty; the anthropic-skills namespace is the app's internal bundle, not a user-installed plugin.

Steps to reproduce

1. Manage a handful of skills through claude.ai → Settings → Capabilities → Skills (no local ~/.claude/skills).
2. Update the desktop app to 1.40609.0 (or launch it fresh on that build).
3. Open any chat and type / followed by a skill-name prefix (e.g. /odoo).

Expected: each skill listed once (bare name, or at least deduped against its anthropic-skills: alias).

Actual: two entries per skill: odoo-api and anthropic-skills:odoo-api, interleaved in the same menu.

• #90858 — same build, adjacent area: reports namespaced anthropic-skills:* entries missing from new-chat autocomplete, and notes in passing that bare + namespaced entries do not dedupe. This issue is specifically about the permanent double-listing for account-managed skills.
