[BUG] Claude code is talking non sense, making up problems that are not really problems and add totally unrelevent thing in it's thought process

What's Wrong?

Claude code is talking non sense, making up problems that are not really problems and add totally unrelevent thing in it's thought process. It also keeps creating memory so prevent the same mistake but that doesn't work. The proof is there are a lot of similar memories. If the memory really works, why it keeps adding the similar memories. The memories are just there, useless.

What Should Happen?

Get to the point. Ask question if not clear. Don't assume things unless it's obvious.
