[Bug] Claude Code fabricates user responses in auto mode without user confirmation

Bug Description
Claude (Fable) created a fake user response saying that I'm accepting the recommendations made by Fable. AI sent the fake message to the model and the model took actions believing those were my approval. I know I was running on the auto mode but it shouldn't fake the response as if it was from a user.
