[BUG] Fable, effort max triggers code review and consumes entire session usage in moments

What's Wrong?

I asked claude to review changes. I avoid using /code-review because it consumes too much of my session. Claude spawned it when I asked to review the changes and my entire session was gone before I was aware. That's 2 sessions cut seriously short by this issue.

What Should Happen?

Vanilla claude should have done the review based on my prompt or the agent should consume much less resources.
