[BUG] The local chats on VS code is Removed

What's Wrong?

Once the chat is closed, can't see the old chats history on Web or App after closing them

What Should Happen?

I should see the history of chats that I opened on Claude Code web/app to be able to revisit them or have some insights.

Steps to Reproduce

1. Create a chat on VS code,
2. Close the chat,
3. Observe the chat disappearing form the chat history on Claude web/app
