[BUG] Claude code version npm-global (1.0.128), vs code plugin, constantly disconnected and request for re-login

What's Wrong?

while using the latest claude code version - npm-global (1.0.128), in VS code.

i noticed it frequently disconnected and request me to relogin

What Should Happen?

Claude code should not request for the relogin

Steps to Reproduce

1. update the VS code plugin to the latest version
2. restart the extension
3. launch Claude code
4. the Claude code will be prompting for reauthentication
