[Bug] Agent fails to follow user instructions consistently

Bug Description
the agent do not follow my directions. I do not know what happened but the performance of understanding is very low. Especially for simple tasks, i am struggling to handle with opus or sonnet. plase check my chats. guide me if i need to change my method to talk with claude code. thank you
