[BUG] Code review agenst stall out completely

What's Wrong?

When I run /code-review <PR-number>, the agent will spin up a code review bot, and that bot will spin up worker bots. The worker bots will finish, but the code review bot never finishes so it never gives me my review.

What Should Happen?

Should properly tell the secondary code-review bot to continue and give a review back to me.

I was very disappointed because I reported it about a week ago through Claude Code itself and not only has it not been fixed, I also haven't even received a response from the Claude Code team.
