[Bug] Claude Code fails to identify application errors and code quality issues

Bug Description
I've spent the last couple of weeks getting application with obvious mistakes and lacking functionality. Claude doesn't notice and even describes the errors as "features" and while I'm such a huge fan of Claude I'm close to the end. It's possible to get the results I need, but the friction is crazy and the product has really regressed.
