[BUG] Session loses all MCP servers randomly in the middle of a session

What's Wrong?

When I start a session, the MCPs load fine. But later in the session, randomly, they are gone - when I use /mcp it says no servers are configured.

I have to exit and continue for the servers to be reconnected.

I cannot find any pattern around when or why they disappear. I tried using --debug but I see nothing in the logs around disconnection, CC couldn't find anything either. I thought it may be compaction but it wasn't.

Let me know how else I can help troubleshoot.

MCPs should not disappear during the session.

I can't provide a fixed series of steps. it just starts with the mcps and later in the session they're gone.

i run several parallel instances, usually 3-4 at a time.

Claude Code v1.0.126, but i was on 2.02 and this was still there and I've had this issue for many versions
