[FEATURE] Setting to disable auto-attach active document

Problem Statement

Claude code v2 added an auto-attach feature for the active document. This is an annoying feature so it should be possible to disable it by default. Currently I can only disable it for the active session, and whenever I start a new session it's re-enabled and I have to remember to turn it off to prevent passing useless context to the model.

Proposed Solution

Add an option similar to copilot:
