[Bug] Claude Code makes incorrect assumptions and stops at first match without thorough verification

Bug Description
Guesswork, assumptions, required for me to always be on my toes and pushback most of the time due to wrong assumptions, stops checking further at the first hit, does not read the notes on a subeject unless clearly instructed, difficult to wrok with.
