Unify Claude Code session history across IDE integrations

Description
Sessions started in one IDE integration (e.g. Cursor) don't appear in another IDE's session/history picker (e.g. VS Code, JetBrains), even when working in the same project. All transcripts are already stored locally under ~/.claude
