[Bug] Subagents idle without supervisor intervention and off-plan code generation despite verification

Bug Description
A couple of challenges: 1) Often subagents go idle for hours and the supervisor agent does not poke them, 2) Often it goes and build things that was not in the plan, even after verfiing for then 2 to 3 times, the result was 100% not on the plan.
