[Feature Request] PreToolUse "Ask" return should include hook message and tool context

Bug Description
"Ask" return from PreToolUse doesn't show any context - or at the very least it doesn't in Auto permission mode. It doesn't show the message from the hook that raised it, nor does it show the tool being called or what the operation in question is.
A user cannot assess whether to allow or deny when they have no idea what they're giving permission for.
Solution
Allow raising custom messages with tool context for "Ask" returns.
