[Bug] Claude Opus 5 ignores task scope and performs unnecessary modifications requiring additional work

Bug Description
Another example of the opus 5 missing context and not doing what it was tasked. More specifically, doing way more than it was tasked, but not in an overachiever sort of way. More like, I know you told me to take out the garbage, so I started replacing your carpet, but only the parts of replacing your carpet that force you to do WAY more work." I have been having these issues all day with Opus 5. I probably had them last week too, but i would just switch to Fable 5 when it was annoying, but not soul crushingly frustrating.
