[Bug] `/code-review` exhausts entire usage allowance due to runaway recursive behavior

Bug Description
I'm a Claude Max 5x subscriber and I'd like to report what appears to be a Claude Code /code-review runaway-usage bug. On two separate occasions, I started with a fresh five-hour usage window and ran /code-review on a GitHub pull request. The review appeared to hang for an extended period, and then my entire five-hour Max usage allowance was exhausted. This has now happened twice, each time starting with a fresh usage window. This appears potentially related to the known Claude Code issue involving recursive/fan-out behavior in code review: https://github.com/anthropics/claude-code/issues/77361 Could you please: 1. Investigate the usage generated by these code-review sessions. 2. Confirm whether this was caused by the known code-review/subagent issue. 3. Restore or credit the usage that was consumed because of the bug, if possible. I'm on the Max 5x plan, so losing an entire usage window twice due to a runaway review workflow has had a significant impact on the service I'm paying for. I can provide the Claude Code version, approximate timestamps, PR details, and usage information if required.
