[Bug] Excessive file searching in implementation mode despite clear plan context

Bug Description
A task was planned using the plan mode on how exactly should the implementation for the given task should take place. I usually use higher effort for planning and lower effort for implementation. When I switched to medium effort on the same model, Claude went on searching / greping random files - initially I though it was closing few gaps - but when it continued with the same approach - I cancelled the job and asked what are you searching for when the implementation plan is clear. It accepted the mistake but also wasted around ~16 tokens. It wasted ~14K token before as well when I had asked to review a PR, it searched random files until I cancelled and mentioned the ask to review a PR is pretty clear. ~14K was much before and part of different session
