[Bug] Goal execution loops indefinitely after completion with Opus 5

Bug Description
Since the release of Opus 5, I've been running into a bug in my Claude Code sessions. My Claude Code installation is up to date.

The issue occurs specifically when using /goal. Claude proceeds as expected and begins implementing the task, working through it to completion. Once all tasks are finished and the visual todo list is fully checked off, the screen flashes and displays a completion message with its findings.

However, this final report then disappears, and Claude immediately re-runs the same goal from the start. This repeats in a loop until I either exit the session or manually run /goal completed.
