What happened

The user corrected the assistant. Captured automatically at the moment of the
correction, so the assistant's own account of events isn't in the loop.

Told the user to report the problem with /bug. That command does not exist in the Claude Code VS Code extension ('/bug isn't available in this environment') and 'claude --help' lists no bug subcommand. Stated from memory, one turn after being called out for exactly this pattern.
