[Bug] Workstream loses track of in-flight agents and work with repeated queries

Bug Description
the workstream is repeatedly losing track of in-flight agents and work, my instructions to create standing rules for how often terminal feedback should be given and even just asking what the current status of in flight work is are being flagged and model downgraded
