[Bug] VSCode Extension: MCP Access Failure Despite Terminal Functionality

Bug Description
in the vscode extension, it has none of my mcp's even though I have access to these mcp's via claude code in terminal. it says acessing the mcp's "failed". i stopped and restarted it and I also stopped and restarted my claude code in terminal, this behavior is consistent
