[Bug] Persistent File Modification Error During Code Editing

Bug Description
i am getting edit file error suddenly again and again "Error: File has been unexpectedly modified. Read it again before attempting to write it." even the Opus modal is acting like too noob before it was working best now its too noob like need to teach everything again and again same thing
