[Bug] /clear command doesn't immediately clear session, requires explicit exit

Bug Description
When sending /clear the session is not directly cleared, you need to explicitely exit. sometimes i prompt further and after some time while my model is working on an response the session is then cleared. occured since today on multiple models in claude code
