[Bug] Claude Code ignores user instructions and deviates from requested behavior

Bug Description
It is not listening to instructions and trying to do it's own thing too much. It thinks it knows better, does not fully follow instructions, constantly breaks out of loops, etc. It has introduced just as many bugs as it has fixed. It says "my bad" and tells me it is it's fault, but that is still tokens and time I am paying with because it would not fully follow instructions.
