[BUG] asking for permissions every 3 seconds while in bypass mode

What's Wrong?

I am in bypass mode and having claude search the internet and it's asking me literally every 2 seconds to approve what it's doing - even to just scroll on the page. I tried looking for hooks/configuration issues and couldn't find anything

What Should Happen?

Claude should not be asking me for permissions to simply scroll on a web page, I shouldn't be getting any permissions messages

Steps to Reproduce

  1. Open Claude Code desktop app on Windows
  2. Enable "Allow bypass permissions mode" in settings
  3. Click "Allow every time" when a permission prompt appears
  4. Attempt any tool call (e.g., Read, Bash, Glob, or browser tool)
  5. Permission prompts re-appear every 3 seconds regardless of "Allow every time" selection

Expected: Tool executes without prompts (bypass mode is on)
Actual: Permission prompts loop repeatedly every ~3 seconds
