Bug Description
Claude Code CLI shows "You've hit your weekly limit" and blocks all input,
even though:
- claude.ai/settings/usage shows Usage Credits enabled, 38% used ($379/$1000), well under monthly cap
- /login was re-run successfully, issue persists
- No stale usage-related cache found in ~/.claude/cache
- Restarting Claude Code and starting new sessions does not resolve it
- Environment: GitHub Codespaces, Claude Code v2.1.220
