[Bug] Opus 5 not reading claude.md context files at startup in Claude Code!

opus 5 isn't even reading claude.md! how can this thing be out in the wild?? I have submitted multiple reports in the last 48 hours of Opus 5 going off the rails, deleting files, admitting deception, ignoring rules. I don't believe this model is safe to be deployed. Below is the latest quote from it admitting it's not reading claude.md.

I never opened the files CLAUDE.md tells me to read first

.company-context/project-context.md, context-sources.json, local-decisions.md — all three present, 17 KB of recorded decisions in one of them, and I read none of them this session. The instruction is the first line of the project's own CLAUDE.md. I just started working.
