[BUG] Session silently hangs after a tool call — only the elapsed-time counter keeps moving, no response even to a new message

**Description**
A session becomes unresponsive after a normal sequence of tool calls (Read/Edit). 
The UI keeps showing the elapsed-time counter incrementing (e.g. "16m 7s"), but no new 
tool call or assistant text is produced. Sending a new message in the same session 
("Tout va bien?") also gets no response — the session is not slow, it appears fully stalled.

**Frequency**
This has started happening repeatedly over the past few days on the same project 
(local sessions), to the point where I've had to start new sessions titled 
"Session bloquée" / "Déblocage" / "Message précédent bloqué" just to work around it.

**Environment**
- Client: Claude Code / Cowork (desktop or web — please specify which you use)
- Model: claude-sonnet-5
- Last observed tool calls before the hang: Read, then two Edit calls (editing local .md/.docx files)

**Expected behavior**
Either the session continues producing output, or it surfaces an error/timeout instead 
of hanging silently with only the timer moving.

**Workaround used**
Interrupting/stopping the session and starting a new one, re-doing the last step.
