[Bug] Anthropic API Error: Permission mode changed false positive in opus plan mode

Bug Description
claude code in opusplan mode constantly asking " Permission mode changed while the auto-mode classifier call was queued — falling back to manual approval under the current mode", although it is not true that permission mode is changing. I've updated Claude Code today to 2.1.220 , yesterday it did not have this issue. The problem persists over different projects.
